Step 1 (Figure 1): General exam and treatment plan

Before proceeding with dental implantation, the following evaluations are required:

General health examination:

  • Generally, if a patient is eligible for tooth extraction, he or she is eligible for dental implants.
  • Dental implant contraindications include:
  • Consumption of more than 10 cigarettes/day.
  • Unmanaged diabetes.
  • Children under 18 years old.
  • Jaw bone that has been exposed to radiation due to previous cancer treatment

Oral health evaluation

  • Before dental implantation, it’s required to diagnose and treat any existing oral diseases.
  • More attention should be paid to diseases in the areas of the missing teeth or adjacent ones.
  • Tartar removal, periodontitis treatment, and pulpitis treatment are all recommended.

Evaluation of the gap

  • Check to see if there are enough bone or gum to sustain the implant and if the width of the gap is large enough for the implant.
  • Images of the roots, including panorex imaging and CT scans, are important for evaluation.

After the exams, using your results, your dentist will come up with the most suitable treatment plan for you.

Treatment plan:

  • How many implants are needed?
  • Do you need surgery to increase bone and gum thickness?
  • Choose a suitable technique: one-phase or two-phase?
  • Choose the suitable type of restoration: Crowns, bridges, or full-jaw restoration?
  • When to carry out restoration: Immediate restoration, early restoration (from 1 day to 4 weeks), or late restoration (3 to 6 months).
  • What type of temporary denture to use during the waiting period?

Step 2 (Figure 2): Bone graft and implant placement

  • The procedure only requires local anesthesia at the implant site.
  • Incision into the gum may or may not be needed.
  • Drills of different diameters will be used to enlarge holes (The drills will be picked depending on the diameters and sizes of the implants).
  • The implant will be placed into the bone and covered with a special cap. Bone graft will be carried out if needed.
  • Gum incision (if any) will be closed with stitches.
  • Osseointegration usually occurs within 2 to 6 months.

Step 3 (Figure 3): Placement of the healing cap

  • Although this is a minor surgery, you will still need local anesthesia.
  • Your dentist will cut into your gum and replace the implant cap with a healing cap.
  • If it’s a front tooth, a gum graft may be necessary for cosmetic reasons.
  • Finally, your gum will be closed with stitches. After 2 to 4 weeks, restoration can be carried out.

  • A model of the implant will be made with a special tool.
  • The timeline for follow-ups and the duration of each visit vary, depending on the type of restoration.
  • Restoration above the implant will restore your chewing function and appearance.

Once the implant is in place, oral care is extremely important. Dental implants are just like your real teeth. If you want them to stay healthy, you have to take good care of them.

Here are some points to keep in mind when it comes to post-op oral hygiene:

  • After every meal, brush your teeth thoroughly with an interdental brush which has a small and pointed head (designed for implants) to clean the gaps and the abutment.
  • Floss regularly, particularly after meals to clean around the implant, under the bridge, and the front and back of the abutment.
  • Avoid hard or chewy foods such as ice cubes, bones, coconut candies, etc.
  • Rinse with mouthwash to reduce bacteria buildup, creating a clean environment for the rest of your teeth. It’s best to use a saline solution that tastes just as salty as an average broth.
  • Limit your consumption of cigarettes. Meanwhile, keeping your nighttime teeth grinding habits is also important to make sure your implant lasts a long time.
  • Follow-up visits: You should visit your dentist twice a year.
